Parental Waiver, Release of Liability, and Consent Agreement

Pat's Pizza The Concept Cuisine

Please read this agreement in full before signing up. By checking the

acknowledgment box on the signup form, you agree to all terms below on

behalf of yourself and your child.

1. Nature of the Program

This program is a voluntary, educational, family enrichment activity.

It is not employment, an internship, or a work placement. Your child

will receive no wages or compensation, has no scheduling obligation,

and no obligation to return for future events.

2. What Your Child Will Do

With you by their side the entire time, your child will greet guests

at your own table, carry already-plated food from the kitchen pass to

your table, and briefly check in with your table about the meal.

3. What Your Child Will Not Do

Your child will not enter the kitchen, handle raw food or hot

equipment, use knives, touch cash or the point-of-sale system, or

serve any table other than your own. Your child will never be left

unsupervised.

4. Parental Supervision

You agree to remain physically present with your child at all times

during the program. Restaurant staff are there to guide the evening,

but you remain responsible for your child's supervision throughout.

5. Assumption of Risk

A restaurant environment carries some inherent risk — things like

slippery floors, hot food, or crowded walkways. By participating, you

accept these ordinary risks on behalf of your child.

6. Release of Liability

To the fullest extent allowed under Pennsylvania law, you release Pat's

Pizza The Concept Cuisine, its owners, and staff from claims related to

your child's participation, except in cases of gross negligence or

willful misconduct.

7. Indemnification

You agree to hold Pat's Pizza The Concept Cuisine harmless from claims

arising from your child's participation, to the extent already released

above.

8. Medical Treatment

If you cannot be immediately reached during an emergency, you authorize

staff to seek emergency medical care for your child. You remain

responsible for any related medical costs.

9. Photo & Video Consent

Photo/video consent is requested separately and is entirely optional —

your choice here does not affect your child's ability to participate.

10. Governing Law

This agreement is governed by the laws of the Commonwealth of

Pennsylvania, with any disputes handled in Chester County, PA courts.

By checking the acknowledgment box on the signup form and typing your

full legal name, you confirm that you are the parent or legal guardian,

that you have read this agreement in full, and that you agree to its

terms.
